NEVER AGAIN will I have the hospital transfer my mother here again, STAFF IS terrible no communication at all. Every question I’ve asked i get the same BS answer, oh I have to ask the Dr. My mother missed appointments cause of this staff. Among other ISSUES that I have with this Facilitie. Don’t have your family brought here if you love your family.
Very Professional and Friendly Staff. Attentive to patients. Very clean and orderly place. I feel very satisfied with the treatment my friend is getting here.

I haven’t experienced Trellis enough for a 5 star review, but on this first visit I experienced a clean facility, with prevalent rehab activity, and a very friendly, helpful staff.
The staff was very friendly and sincere after my father passed. My dad could be very difficult at times but the staff was very patient with him.
This was a negative review about my fathers care at Trellis. After experience truly abhorrent care for my mother at another facility, not Trellis, I can see now that it is not Trellis, it is the very broken healthcare for profits system we have in America that leads to care concerns: So considering this I would now recommend Trellis based on my opinion that there are way worse options. Just stay involved with your loved ones care team for best outcome.
Lorraine seems to be happy and slowly healing. She’s more patient than I am. I always have a problem with the food served. She didn’t touch it so we left. Clean and friendly.
Took very good car of our friend while he was in for help recovering from seizures – everyone was very professional and helpful.
In the life you can confront a difficult health situations 😑 and I need to says it was a bless to have this place in my options for my recovery .. the employees were so empathetic to me in those terrible times, and make those people feel loved and valued and their motivation helped me to get better and fight for me and family .. I just say thank you for being there for me 🥹🙏🏼.. there is more people enjoying this amazing place.. some of them don’t have family close and all of this lovely stuff helps them to keep up 😇😇😇 THANK YOU TRELLIS CENTENNIAL!
Called three different times, got sent to voicemail. No one wants to pick up the phone or let me speak to someone.
My family was there and had a very poor experience. She was not attended for on several occasions when she called a nurse it took them sometimes 20 minutes to come and sometimes not at all. There are also some uncaring and even abusive nurses in the facility . Highly recommend you seek care other places if possible . Not all staff is bad but they definitely need some checks and balances especially with the case management. Have your family wear a camera so you can know how they are being treated at all times
In my opinion, this place likely contributed to my grandmother’s decline and eventual death. She had cancer and a broken arm, and yet every time I’d visit I would continuously see the nursing staff handle her broken arm very roughly, to the point that she’d be evidently in pain when being moved. Beyond that, the staff were highly inattentive, disrespectful, and provided false information. They told us they would monitor her situation and would help with scheduling appointments, but even after weeks in their facility, she would be continuing to decline with no sign of any appointments, improvements, or sense of urgency. During one point when she was at a very significant decline, when we’d question the nursing staff about bringing her to the hospital, they mocked the idea. Specifically, when approaching the idea of bringing her to the hospital, one nurse stated in a mocking tone, “you’ll be waiting a long time.” How is that even remotely appropriate?? Of course we’d need to wait, it’s a hospital, but the staff at Trellis would prefer to keep a declining elderly woman in their facility than allow a more skilled facility to potentially improve the situation. Aside from their negligent care, the staff were also consistently uninformed and often unable to contact. Several times when we’d attempt to call, we were hung up on or told X person was unavailable, even if we’d call day after day. And then, even though it was CLEARLY stated numerous times that she had cancer and a fractured arm because of it, the nurses and doctor himself would act as if her injuries were superficial, and would scoff at the idea of cancer, even though a cancer specialist confirmed it was such. We spent over a month in this facility, and that was a month for my grandmother to be neglected, poorly cared for, and treated disgustingly. In that time, we could have received proper care for her, brought her to doctor appointments or hospitals ourselves, and maybe we could have had more time with her. If you are considering bringing a loved one into this facility, I urge you to think again. Your loved one may be treated like an animal, or at the least will receive significantly better treatment elsewhere. Trellis needs to fire every single one of its current employees, because they are a stain on the medical field.

My mom recently has been at Trellis Centennial and everyone one of the staff I have come across have been exceptional. The case workers, Kristi, Annabelle, Laura and head nurse Lanny are responsive as well as showed that they truly put the care of my mom first which is comforting and puts your mind at ease. I am in another state navigating the care of my mom and I have interacted with several providers/staff, they have by far have been the most efficient as well as kind to work with. I appreciate them all and I am sincerely grateful for their help.
This facility is amazing and the care that they gave my mother deserves more than five stars. All of the staff showed compassion, Understanding and love for The work they do.

They take good care of their patients. My father has been in for rehab and they really try to help the patients recover. They provide good care for the folks that are weak and from surgery.

Everyone at Trellis has been great! They are helping Pop learn to do things for himself again, which is difficult, after a stroke. I wish I could name and thank them all! Sometimes Pop needs a little tough love to motivate him, and they encourage him to do some things for himself, which is good. Thank each and every one of you!!!

I have to say I was very nervous preparing to go here but my first day was very comfortable. As time went on the PT’s and OT’s were amazing. When it came to listening to where you wanted to be they made sure to get you there and always moved with care. They made sure you had a goal and helped you achieve it at your pace but gave a lil nudge when needed. They never left your room until they made sure you didn’t for need anything they can get you 😂. All the PT’s and OT’s are amazing look out for ANDREW, SABRINA, RAYN, CHARLENE, SHAY, RAYMOND, JEANINE, JERMAINE and RUSTIN just to name a few. The best CNA’s i have ever had, they were always attentive and super helpful always asked questions and made sure you had everything you needed.My fav CNA’s hands down ANIKA, EMILY, NATALYA, SYMPHONY, JOSH, KORTNEY, DOMS and KAI these beautiful amazing people made sure I was taken care of and never needed for anything. The nurses were cool did they job and tried their best to get to their patients in a timely manner, my fav Nurses were HESPER, PETER, HELEN and there was another who I made a Lei for her nieces graduation 👩🏽🎓 but definitely 💯 my favs. I just wanna thank all the staff for taking great care of me and even my family when they visited. I have so much ALOFA (love) for you all and keep you all in my prayers 🙏🏽 because we became family while I was there. I love this place and would recommend them to ANYONE & EVERYONE. It would be nice if each department (ie. Nursing, PT, dietary, house keeping) wore different colored scrubs or lanyards to identify themselves. That way visitors would not bother staff for requests they can’t help and avoid wasting their time. It was nice that my friend got a flower arrangement for the 4th of July. Great that there was a therapy dog visiting today (10 July).
