I’m actually shocked they aren’t rated higher overall. I’ve been here a week and have no complaints. The staff responds fast to all requests, the food is absolutely amazing, and having a private room with your own a/c is needed in this city. If you or your family are needing care I would suggest coming here for sure! The PT facility is amazing for rehab with experienced therapists. The OTs have made me feel like I will keep improving! All the RNs and CNAs make the stay easy. Even the support staff is extremely friendly. Thank you Trellis!

Was told many different things by many different staff no one knew a simple answer. Like when are they going to help my mom to the sink to brush her teeth. I knew it had been a week because the toothbrush wasn’t even opened yet. I was told my mom would have an OT that would help her to go to the bathroom, brush her teeth daily needs along with speech therapy 5days a wk. She got none of that. They kept her on a catheter when she could shuffle-walk. Mainly they kept her in a wheelchair. They just didn’t want to bother To help them to the restaurant she wore a diaper and a catheter. They wouldn’t let her take a shower which she could sit on the bench and she had shower shoes that were non-slip. She wanted her hair washed and it had been a month. Bed baths were not Rehabilitation they treated my mom like she was a baby. So humiliating. She started to get angry. An elderly patient was calling for help, someone help me. She was in a wheelchair outside her room.. The patients at the table were in distress so I went to the nurse and called to them. They’re actually standing right there in the same room. They chatted for a minute about the patient and then one of them finally went over to her. I heard one of the nurses say that’s …. and she always does that😠 What? Want to go in her room? To me it seemed the only time anything would get started is when I would complain about it or ask questions.

Amazing group of ladies at the front desk. Very knowledgeable, compassionate, they are angels.

They have the bare minimum. They don’t even have a toilet seat riser. I bought one and gave it to them. The riser they use is something with a hole that slides over the toilet. Hard to get in and out of depending on your situation. Bed protect is a cloth blanket or chuck. No disposable chucks here. Understaffed. Wait time when calling to go to bathroom because you need help can be up to and longer than. 30 minutes. For the most part the staff is wonderful. But some could care less and don’t have your best interest in mind. I’m listening to a staffer argument with a resident and escalating their conversation to a confrontation. The foods really good

I visited a church sister. It was very goid to hear that she really likes the people she has met here. Also she feels confident in the care she is receiving. I was only a visitor.

Trellis Centennial was amazing for my family through the entire 4 1/2 months both of my parents were there. They went above and beyond what a Skilled Nursing Facility normally does. All of the staff were amazing and always very helpful no matter what obstacles we came across. They were caring and compassionate not only to my parents, but to myself. I can not express my deepest gratitude to this facility and it’s staff.

This facility is a great place. The nurse Zhai, and Erica, the CNA Natayla, the therapist Kristy, Kelly were great to me. Thank you Kristy for all your help. It was a wonderful stay!

Trellis Centennial is truly a 5 STAR facility! My father recently was a patient at Trellis and the care he received was truly the best, starting with Kristy admissions/marketer is truly so caring and beyond helpful, The Nursing staff and the Therapist were everything in his journey of healing and the reason he has made such great process, to finally go home after being in the hospital for 68 days. He loved the food, his Case Manager Ashley was great, the CNA’s took such great care of him and everyone in the management team was so supportive. This facility is beautiful and the private rooms were a huge bonus to have his own space. I’m so grateful to everyone at Trellis Centennial for providing a safe, comfortable and loving environment for my Dad to heal after having PNA. I highly recommend this facility to anyone who is looking for their loved one! ⭐️⭐️⭐️⭐️⭐️

I saw several positive reviews about this facility, so I chose it for my mom after she was discharged from the hospital. Shortly after she was admitted, I was told she had developed a pressure wound and ended up staying there for 40 days. I’m not sure if that length of stay was normal, but throughout that time I was repeatedly told that she was getting better. Unfortunately, she never returned to the level of responsiveness she had before. She rarely talked or responded, which was very concerning to me. I also expressed concern about the Medicare copay. I was told not to worry because the copay would be taken care of. However, the day after my mom was discharged, I received a call from the billing department requesting payment for the copay. I have dealt with several rehabilitation facilities over the years, and this was by far the worst experience. My advice is not to rely solely on verbal promises. If you are told something important regarding your loved one’s care, insurance coverage, or billing, ask for it in writing or keep a record of the conversation.

My Sister , Virginia has been there for 3 weeks , the only negative comment she has had is about her requests for the food menu to be selected by the patients . As long as there’s not a diet restriction the food selections should be left up to the patients . There are selections that seem to be the same everyday. A little variety based upon taste and availability might help the long stays due to rehab and PT based upon surgeries . Why can’t the patients be given more choices of food 3 times a day like other successful hospitals? Other than the above her stay has been ok.

This place looks good at first but don’t be fooled ,the second my primary insurance ran out they threatened me with arrest if I didn’t leave and would not wait for my Medicare review they sent me out without medication and now it’s a real struggle to get them ,heart attack just a few weeks before ,no compassion

While it is a clean facility and the nursing staff are wonderful i found it more of a nursing home than a rehabilitation center. One afternoon upon entering i saw them lining up patients in front of a television to watch a movie. Like they do in nursing homes as an activity. My biggest complaints haver to do with the case manager, James. He was not competent in doing his job at all. I was to the point of pulling my MIL out and taking her back to the hospital, because he was not scheduling her doctor’s appointments as needed. Rose stepped in and took care of business. She was extremely helpful and understanding. Then, there is the booking billing agent. Who will call you and expect full payment at that moment. I’m sorry but depending on the insurance coverage that can take from hundreds to tens on thousands of dollars. She is relentless. We asked her to bill us. She sends an invoice and calls again for payment. My MIL hadn’t even been released yet! If we can’t pay on demand are they going to hold her captive for ransom? In general I found that staff and patients are given the bare minimum. Which is really not acceptable. I’m not saying that they need every gadget or to of the line, but I see no areas for occupational therapists to work with patients for tasks such as grocery shopping and other real life tasks. I would not recommend trellis to anyone

Worst place ever I sat in my own waist. 5 hrs till someone cleaned me up. Night shift over a hour to get to ya.i would sooner die before going back their horrible place!!!!!

NEVER AGAIN will I have the hospital transfer my mother here again, STAFF IS terrible no communication at all. Every question I’ve asked i get the same BS answer, oh I have to ask the Dr. My mother missed appointments cause of this staff. Among other ISSUES that I have with this Facilitie. Don’t have your family brought here if you love your family.

I haven’t experienced Trellis enough for a 5 star review, but on this first visit I experienced a clean facility, with prevalent rehab activity, and a very friendly, helpful staff.

The staff was very friendly and sincere after my father passed. My dad could be very difficult at times but the staff was very patient with him.
